The Village of Candle Lake is looking for a consultant or firm to develop the operating model for an Aging in Place initiative. The work includes designing service delivery frameworks for transportation, community navigation, and in-home/community supports, plus building the systems, processes, and operational plans needed to launch the program. This is foundational work to enable the community support initiative to start and function effectively.
